When is case management week 2018




















This truly brought together the team for the benefit of the patient as well as the facility. It was a win-win situation and the case managers were better known for what they could do. As a case manager on the payer side, I worked with one of the first West Nile Virus cases in the United States that was totally debilitating to the patient.

They ended up having to sell their home, as she was in the hospital for months and then was transferred to a rehab unit until she was able to walk again. At times, the husband would just call to talk — the case manager was a safe place to cry, complain, and voice his frustrations without having to worry.

This year-old woman and her husband became my family by the end of her care. As a case manager, I was able to make sure that she received the care that she needed within the bounds of her provided benefits — however creatively done. Local education, recognition, and awareness events will be held across the country to recognize this special week. At this time, the ACMA encourages home care agencies , assisted living facilities, hospitals, and other health care organizations to recognize and honor the importance of case management professionals as they dedicate their careers to serving patients and their families.

In honor of National Case Management Week, this program works to provide case management professionals with an opportunity to provide a gift that honors an individual, chapter, department, or organization involved in health care. We encourage case managers to remember and honor those who have made an impact in their life and career as well as those who make a difference in the lives of patients and families they serve.

All proceeds from this fundraising effort will support the ACMA. ACMA is a c 3 non-profit organization, which makes your donation tax deductible. If you are unfamiliar with case management, it is a collaborative practice in hospitals, rehabilitation facilities and other healthcare organizations that includes physicians, social workers, patients, nurses, other practitioners, and caregivers in the community.

It encompasses communication and regulates care through effective resource coordination. In conjunction with National Case Management Week, we asked for nominations for influential case managers who deserve some extra recognition. We were looking for professionals who are passionate about their work and go the extra mile for their patients. We found all that and more. We had over 40 nominations from hospitals and community care programs across the country.

Case managers were nominated by colleagues, supervisors and even former patients. We have loved connecting these incredible people throughout the week and as National Case Management Week comes to a close, we want to recognize a few of the outstanding case managers and will start highlighting their stories on our site. Our first profile is of Christine Aguilar, a case manager at the South Texas Rehabilitation Hospital, who started a free community support group to help stroke survivors cope with the emotional side of recovery.
